On 19 October 2024 the World Medical Association (WMA) adopted the 10th revision of the Declaration of Helsinki on Ethical Principles for Medical Research Involving Human Participants. This is likely the most significant revision of the Declaration since 2000, as it reflects a determined effort to adapt ethical standards to contemporary challenges in medical research.
The major revisions focus on:
• Participant-centred respect and protection: this includes acknowledging participant vulnerabilities, promoting community engagement and ensuring informed consent through more inclusive, participant-focused language.
• Research value: including the pursuit of “individual and public health”, encouraging scientific rigor and integrity, and guarantying the distribution of benefits, risks and burdens.
Notably, the term “subjects” was replaced with “participants” throughout the document. This change highlights the value of individuals, viewing them as active partners in the research process rather than passive subjects.
Moreover, an expanded definition of vulnerability is introduced and recognises that it can be influenced by a variety of conditions that may change over time or across settings. The updated guidelines now highlight the ethical responsibility to carefully balance the inclusion and exclusion of vulnerable groups, since excluding them from research could in turn perpetuate their health disparities. Where vulnerable populations are included in research, they should receive tailored protection and specific forms of support.
Another key update concerns Ethics Committees: the new version of the Declaration states that in cases of international collaborative research, the research protocol must be approved by both sponsoring and host countries. This has a significant impact on research on rare diseases, as studies in this field are typically multi-centre and multi-national.
